Important things to do before you download or Print your annual evaluation form.

The college of education students who are now offering their national service has been asked to submit their annual evaluation form

The NSS has released the annual forms for colleges of education students now offering their national service in various schools.

Our checks indicate that the annual form has appeared on the dashboard of all NSS personnel (college of education students, class of 2019).

Personnel are however required to take note of this vital info before continuing to download the form.

How to check and confirm your information

Log on to portal.nss.gov.gh
Select sign in as personnel to check 2019/2020 posting
Enter your email address and password used in registering your NSS.
Click on the login in, we advise that, you select the remember me button for faster login next time.
In your dashboard, you will see two new tabs
Details confirmed by me(Once you click on this tab, it means you have confirmed that all the details on the form are correct
Request changes( select this option to make changes to your personal details before they appear on your form
Select save, you will be redirected back to your homepage
How to download / save the annual form as PDF

Log on to portal.nss.gov.gh
Select sign in as personnel to check 2019/2020 posting.
Enter your email address and password used in registering your NSS.
Click on the login in, we advise that, you select the remember me button for faster login next time.
Skip the 1st 4 steps if you are already login after requesting the changes
Select the print annual evaluation form.
Next, you can save the form as in PDF and print it at any time.
Information on the annual evaluation form

All your personal information on the annual evaluation form has been filled already with the exception of the following which you will be required to fill them out yourself

1. Date of assumption of duty
2. Address of the organization
3. The organization’s Ghana GPS address
4. Region of Organization.

NB: Part II of the annual evaluation form is to be completed by your supervising officer

Before you submit your NSS annual evaluation form, you are required to take note of the following.

• Bar Code
• NSS logo
• Your picture
